New Patient Registration

Patients living within the practice boundary may apply to join our list by completing the relevant forms. If you are completing the forms online, please ensure you complete both the registration form and the health questionnaire. You can submit the form online but you will be required to come to the surgery and sign the form before it can be processed and complete your registration. You should also bring proof of identification with you.

Due to NHS fraud, proof of identity will be required; a passport or a picture driving license being the preferred method of supplying proof of identity.

Named GP

All patients at Arbury Medical Centre have a named, accountable Doctor, who is responsible for coordinating their care. Please contact the practice or ask for the name of your named GP at your next appointment. This does not affect your choice of GP in the practice when attending appointments.
